Registering with the practice
We are pleased
to register any new patients who live
within our boundaries.
To register as a patient please ask for
a registration form from reception.
You will need to complete one for each
person registering, and return it to
reception. You also need to provide
evidence of identity and address.
Boundaries:
- North - along
the Houston Road until the Appleton Parkway Roundabout.
- South - along
the River Almond until the river joins Livingston Road.
- East - along
Livingston Road to the Houston Interchange.
- West - cross
country from Houston Road to Appleton Parkway through Mill Roundabout and
down to the weir at the River Almond.
i.e. Includes Howden, Ladywell, Eliburn and
Livingston Village including the Kaims estate.
Any patients who subsequently move out
with our practice boundaries will need
to register with a
new GP practice.
